To use these compliance aids, you should download the file to your computer
and open it with the appropriate program.
These forms are in
Adobe Acrobat format. You must have the
FREE Acrobat viewer to view or print these posters. Click on the link above to
go to the Adobe web site and download the free Acrobat viewer if it is not
already installed on your system.

Injury and Illness Incident Report
To download a form to your computer,
1. RIGHT CLICK on the link.
2. Left click on the SAVE TARGET AS... selection
3. Specify the location on your computer where you want the file to
be placed then click on SAVE.
4. When the download is complete, you may resume browsing. When
you want to view the file later, navigate to the folder on your computer where
you placed the file and double click on it to launch the
Adobe Acrobat Viewer and read the file.
