We received a shipment of controlled drugs with broken bottles.  What should we do?

If the bottles are damaged in transit and discovered when the package is received, be sure to take pictures of the damage IN THE BOX and file a claim with the supplier.  It’s also best to have 2 people witness the damage. 

 A DEA Form 106 – Report of Theft or Loss of Controlled Substances should be completed.  The form can be completed manually and mailed to the DEA, but a better option would be to utilize the on-line version which can be found on the DEA’s web site.  Of course, be sure to keep a copy of the notification form for your records.

Also don’t forget to show the receipt of the shipment on your log but make an immediate second entry subtracting the loss amount with a note to “see Form 106 for details.”
